Vous aurez ainsi plus d\u2019information sur la mani\u00e8re dont les embryons sont cr\u00e9\u00e9s dans le laboratoire d\u2019embryologie et sur les raisons pour lesquelles les embryologistes choisissent cette technique dans votre cas particulier.<\/em><\/p>\n\n\n\n<p>La <a href=\"https:\/\/gravida.com\/fr\/treatments-fr\/fecundacion-in-vitro-fiv\/\">f\u00e9condation in vitro (FIV)<\/a> consiste en la f\u00e9condation des ovocytes par les spermatozo\u00efdes en laboratoire. <strong>Il existe deux types de techniques d\u2019ins\u00e9mination: la f\u00e9condation in vitro classique (FIV) et celle qui se r\u00e9alise par micro-injection intracytoplasmique (ICSI).<\/strong> Chacune d\u2019entre elles est appliqu\u00e9e en fonction des besoins sp\u00e9cifiques de chaque cas.<\/p>\n\n\n\n<p>Avant l\u2019ins\u00e9mination ovocytaire il est n\u00e9cessaire de traiter l\u2019\u00e9chantillon de sperme. Il est recommand\u00e9 d\u2019obtenir l\u2019\u00e9chantillon frais (dans la mesure du possible) et de respecter une abstinence d\u2019entre 3 et 5 jours. Une fois l\u2019\u00e9chantillon trait\u00e9 et en fonction du comptage final obtenu, il sera d\u00e9cid\u00e9 de la technique plus ad\u00e9quate.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">FIV conventionnelle<\/h3>\n\n\n\n<p>Dans le cas d\u2019\u00e9chantillons <strong>normozoospermiques<\/strong>, la FIV conventionnelle est le premier choix pour la f\u00e9condation, \u00e0 condition qu\u2019il n\u2019y ait pas d\u2019autres contre- indications (par exemple, des \u00e9checs de f\u00e9condation pr\u00e9alables). Elle consiste \u00e0 cultiver ensemble les ovocytes et les spermatozo\u00efdes. Pour cela, il est n\u00e9cessaire de disposer d\u2019une plaque de culture dans laquelle les ovocytes seront plac\u00e9s et o\u00f9 ils seront ins\u00e9min\u00e9s \u00e0 une concentration correspondant \u00e0 &gt;100.000 spermatozo\u00efdes mobiles\/ ml par ovocyte, afin d\u2019obtenir une f\u00e9condation correcte. L\u2019ins\u00e9mination est effectu\u00e9e 4 \u00e0 6 heures apr\u00e8s la ponction.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"349\" height=\"255\" src=\"https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/FIV-CONVENCIONAL.jpg\" alt=\"\" class=\"wp-image-8329\" srcset=\"https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/FIV-CONVENCIONAL.jpg 349w, https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/FIV-CONVENCIONAL-300x219.jpg 300w, https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/FIV-CONVENCIONAL-16x12.jpg 16w\" sizes=\"auto, (max-width: 349px) 100vw, 349px\" \/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<h3 class=\"wp-block-heading\">FIV par micro- injection intracytoplasmique (ICSI)<\/h3>\n\n\n\n<p>Pour les \u00e9chantillons pr\u00e9sentant une alt\u00e9ration quelconque (<strong>asth\u00e9nozoospermie, oligozoospermie ou t\u00e9ratozoospermie<\/strong>), <a href=\"https:\/\/gravida.com\/fr\/treatments-fr\/fecundacion-in-vitro-fiv\/\">l\u2019injection intracytoplasmique de spermatozo\u00efdes (ICSI)<\/a> est le premier choix pour l\u2019ins\u00e9mination, car la faible concentration, la faible motilit\u00e9 ou le pourcentage \u00e9lev\u00e9 d\u2019anomalies morphologiques des spermatozo\u00efdes peuvent compromettre la correcte f\u00e9condation des ovocytes. Elle est \u00e9galement essentielle pour l\u2019utilisation d\u2019\u00e9chantillons de grande valeur biologique (tels que les \u00e9chantillons cryoconserv\u00e9s pr\u00e9alablement \u00e0 des traitements de chimioth\u00e9rapie) ou les \u00e9chantillons provenant d\u2019une biopsie testiculaire.<\/p>\n\n\n\n<p>Pour r\u00e9aliser une ICSI, il faut respecter les \u00e9tapes suivantes:<\/p>\n\n\n\n<p>En premier lieu, les ovocytes doivent \u00eatre d\u00e9nud\u00e9s pour confirmer leur stade de maturation. Les ovocytes sont class\u00e9s en fonction de leur stade de maturation nucl\u00e9aire, qui est identifi\u00e9 par leur morphologie&nbsp;:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Ovocytes MII<\/strong>: Ce sont les ovocytes matures. Ils se trouvent en m\u00e9taphase II et le premier corpuscule polaire (CP1) peut \u00eatre observ\u00e9.<\/li>\n\n\n\n<li><strong>Ovocytes MI\u00a0<\/strong>: Ce sont les ovocytes immatures. Ils se trouvent en m\u00e9taphase I et ni le premier corpuscule polaire ni toute autre structure du cytoplasme ne peuvent y \u00eatre observ\u00e9s.<\/li>\n\n\n\n<li><strong>Ovocyte VG<\/strong>: Ce sont des ovocytes immatures. Ils se trouvent en prophase I (diplot\u00e8ne). Le CP1 n\u2019est pas aper\u00e7u, mais une membrane nucl\u00e9aire intacte et bien d\u00e9finie, appel\u00e9e v\u00e9sicule germinale peut \u00eatre observ\u00e9e \u00e0 l\u2019int\u00e9rieur du cytoplasme.<\/li>\n\n\n\n<li><strong>Ovocyte d\u00e9g\u00e9n\u00e9r\u00e9\u00a0<\/strong>: Le stade de l\u2019ovocyte ne peut g\u00e9n\u00e9ralement pas \u00eatre confirm\u00e9, car les structures sont d\u00e9g\u00e9n\u00e9r\u00e9es. Le cytoplasme est sombre et l\u2019ovocyte est en \u00e9tat de n\u00e9crose cellulaire.<\/li>\n<\/ul>\n\n\n\n<p>&nbsp;<\/p>\n\n\n\n<p>Un ovocyte normal est totalement sph\u00e9rique et son cytoplasme est homog\u00e8ne. Le CP1 est bien d\u00e9fini, homog\u00e8ne et l\u00e9g\u00e8rement aplati. Absence d\u2019espace p\u00e9rivitellin (EP) entre l\u2019ovocyte et la zone pellucide (ZP), qui doit \u00eatre r\u00e9guli\u00e8re, d\u00e9finie et homog\u00e8ne.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img loading=\"lazy\" decoding=\"async\" width=\"284\" height=\"242\" src=\"https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/OVOCITO-MII.jpg\" alt=\"\" class=\"wp-image-8334\" srcset=\"https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/OVOCITO-MII.jpg 284w, https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/OVOCITO-MII-14x12.jpg 14w\" sizes=\"auto, (max-width: 284px) 100vw, 284px\" \/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>\u00c0 la suite de leur d\u00e9cumulation et classification, uniquement les ovocytes matures au stade de m\u00e9taphase II et leurs possibles variations morphologiques pourront \u00eatre ins\u00e9min\u00e9s.<\/p>\n\n\n\n<p>La micro-injection intracytoplasmique (ICSI), consiste en l\u2019introduction m\u00e9canique d\u2019un seul spermatozo\u00efde dans le cytoplasme de l\u2019ovocyte. Cette technique n\u00e9cessite un appareillage tr\u00e8s pr\u00e9cis, compos\u00e9 d\u2019un microscope invers\u00e9, deux micromanipulateurs et deux microinjecteurs. Avant la micro-injection, il est n\u00e9cessaire de pr\u00e9parer une plaque sp\u00e9ciale, qui permettra la manipulation de l\u2019\u00e9chantillon de sperme trait\u00e9 et des ovocytes.<\/p>\n\n\n\n<p>Deux types diff\u00e9rents de micropipettes sont raccord\u00e9s \u00e0 chacun des micro- injecteurs. La micropipette holding permet le maintien de l\u2019ovocyte, tandis que la micropipette ICSI capture le spermatozo\u00efde, perce la membrane et d\u00e9pose le spermatozo\u00efde \u00e0 l\u2019int\u00e9rieur du cytoplasme de l\u2019ovocyte.<\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter\"><img decoding=\"async\" src=\"https:\/\/gravida.com\/wp-content\/uploads\/2021\/08\/ICSI.jpg\" alt=\"\" class=\"wp-image-8339\"\/><\/figure>\n\n\n\n<p><\/p>\n\n\n\n<p>La rupture de la membrane de l\u2019ovocyte peut se r\u00e9aliser de deux fa\u00e7ons&nbsp;:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Par aspiration<\/strong>: une fois la micro- pipette ICSI ins\u00e9r\u00e9e dans l\u2019ovocyte, celui- ci est aspir\u00e9 jusqu\u2019\u00e0 la rupture de la membrane.<\/li>\n\n\n\n<li><strong>Par injection\u00a0<\/strong>: le seul mouvement de la micropipette r\u00e9ussit \u00e0 percer la membrane.<\/li>\n<\/ul>\n\n\n\n<p>Apr\u00e8s la r\u00e9alisation de l\u2019ICSI, les ovocytes micro- inject\u00e9s sont transf\u00e9r\u00e9s sur une <a href=\"https:\/\/gravida.com\/fr\/treatments-fr\/culture-jusquau-stade-blastocyste\/\">plaque de culture<\/a> qui est plac\u00e9e dans l\u2019incubateur jusqu\u2019au lendemain, moment o\u00f9 la f\u00e9condation sera \u00e9valu\u00e9e.<\/p>\n\n\n\n<p>Nous esp\u00e9rons que vous avez appr\u00e9ci\u00e9 ce post et que vous ayez pu dissiper tous les doutes que vous aviez.
